This is not a Schengen tourist visa. A National Visa (often called a D-visa) is the pathway used for long-stay purposes, and in the United States it is typically submitted in person at the appropriate Visa Application Centre (like VFS, based on your place of residence), after you book an appointment and prepare your full file, including the correct form, supporting documents, and your application fee payment proof (when applicable). Include the surnames as they appear in your passport.

2. Surname at birth

If you changed your name, either through marriage or a court order, insert your previous family name.

3. First name or given name

Write your first name as it shows in your passport. If it appears in a separate section of your passport (such as middle name), include your middle name here.

4. Date of birth

Insert your birth date in the format DD/MM/YYYY.

5. Place of birth

Write down the city where you were born, according to the information on your passport or birth certificate.

6. Country of birth

Now, write down the country where you were born according to the information on your passport or birth certificate.

7. Current nationality

Insert your primary nationality.

If you were born in a different country from your current nationality and you do hold citizenship there, write it in 'Nationality at Birth'.

If you have any other nationalities, either by descent, marriage, or acquisition, insert them here.

If I was born in the US but raised in England by my British family, how should I fill it?

Fill in current nationality with the British, if you hold a passport from there; in nationality at birth, include the American; and if you have any other nationality, either acquired or through a descendent, include it in 'Other nationalities'.

Your photo and right side of the document must be inserted and completed by your visa officer.

8. Sex (Gender)

Check the box with your gender.

If you are non-binary, check the box with the gender assigned at birth and inform the clerk during your appointment.

9. Civil status

Specify your legal civil status at the time of your appointment.

Even if you'll have a soon-to-be change of status, you must check the box with the status according to your appointment date.

Civil partnerships are only valid when recognized by governmental authorities.

10. Parental authority

Only if you are applying with or for a minor must you fill this box.

It must have all the personal information of the child's legal guardian: surname, first name, passport ID number and nationality, telephone number, address (when different from the minor), and email address. Fill it out in accordance with the legal guardian's passport.

11. National ID number

Insert your ID number.

Please be aware that this is not the number of your driver's license, social security number, or tax number (unless in countries where their tax number is their ID number).

If your country does not provide a national identification document or doesn't have a number, include your passport's number.

12. Type of travel document

Check the box adequate for your document.

Remember, you must have a valid passport for up to 6 months after entering Portugal, to apply for a visa.

13. Number of travel document

Insert the number and letters (if applicable) of your valid passport.

14. Date of issue

Write down the date your passport was granted.

Date format must be DD/MM/YYYY.

15. Valid until

Write down the date your passport will expiry.

Date format must be DD/MM/YYYY.

16. Issued by (country)

Insert the country your passport was issued by, which means, the country of your main nationality.

19. Applicant's home address and email address

Include your full address along with a valid and active email address.

Telephone number

Include a valid and active mobile telephone number.

20. Residence in a country other than the country of your current nationality

If you are living abroad and you are not a national in the country where you are currently living, you must check the 'yes’ box and provide a valid legal status and residence permit ID in the country of your address.

If you are living in a country where you hold a nationality, just check the 'no’ box.

21. Current occupation

Insert your job role.

If you are applying for a university, insert student. If you are retired, insert retired.

Write down the contact info and legal name of your employer.

For students applying for university, if graduated, insert the name and information of your latest educational establishment.

23. Purpose of your journey

Check the box according to your type of visa.

Digital nomads and passive incomers not retired must check the other box and include.

24. Additional information on purpose of stay

Insert: Residence permit.

26. Member State of first entry

If you are arriving in the EU (first entry into European space, if your ticket is for Paris or if you are coming by car) through another country, insert which one here.

If you are arriving through Portugal, insert Portugal.

27. Number of entries requested

For D-level visas, residency visas, you must check ‘two entries’ box.

Fear not, you'll be able to have more entries after converting your visa into a residence permit, after your AIMA appointment.

  • Intended date of arrival of the first intended stay in Portugal

Insert the date of your arrival ticket.

  • Intended date of departure from Portugal after the first intended stay

As a D-level visa applicant, insert a date 1 year and 1 day after your arrival date.

Sponsor information

This area must be filled only if you have an invitation letter from a Portuguese resident or from a local company.

If this is not the case, include only temporary stay information, such as hotels or airbnbs, before heading to your formal address, either in Portugal or in EU (if you are stopping in other country of the EU, for example).

30. Surname and first name of the inviting person(s) in the Member State(s). If not applicable, name of hotel(s) or temporary accommodation(s) in the Member State(s)

If you have a sponsor, include their personal information here.

If you don't have a sponsor, include the name of your first accommodation in EU, here.

Consider sponsors only a family member already residing in Portugal (if you are moving under the D6 visa), a company hiring you (D1 or D3 visa), or an organization bringing you for voluntary work (such as a religious or non-profit role). If you don't fit in a criteria similar to the ones stated, you don't need to fill out this area.

This is only applicable if you already have a hotel/air bnb/temporary accomodation stay booked (for your inital days/weeks in Europe - either if you are entering through Portugal, or doing a stopover in any other EU nation) or if you already have a rental or property purchase contract settled when you arrive in Portugal. If you don't have an accommodation settled for your initial period in Portugal (or Europe), there's no need to fill it out).

The funding source must match your type of visa. If you have a Portuguese bank account with funds, check ‘Other’ and insert Portuguese bank account
